47-year-old Coughlan, who took over from former Saddlers manager Darrell Clarke at the Memorial Ground, helped the Pirates stay in League One 2018/19, before leading the side to fourth in the table before moving on to Mansfield in December 2019.
Coughlan, who made appearances for the likes of Livingston, Plymouth Argyle, Sheffield Wednesday, Rotherham United, Shrewsbury Town and Southend United over a 19-year playing career, left the Stags less than a year later following a string of poor results.
Graham's most recent role saw him form part of Sheffield United’s under-23s coaching department, a position he held until the end of last season.
